## Short answer

For adult weight management, medication is generally considered at BMI 30 or higher, or BMI 27 or higher with a weight-related condition. Meeting that threshold starts a clinical discussion; it does not guarantee a prescription.

BMI is a screening tool that relates weight to height. It can help identify when to discuss weight-management medication, but it cannot answer the whole question: **Is this treatment appropriate for me?**

This guide concerns adult weight management. Semaglutide products have different indications, and a treatment used for another condition may follow different criteria. The [current Wegovy label](https://www.novo-pi.com/wegovy.pdf) also distinguishes injectable and oral products; a familiar drug name alone is not enough to identify the right rules.

## How to read the usual adult thresholds

| Adult BMI | What the number means for a weight-management discussion |
| --- | --- |
| 30 or higher | Meets the usual BMI threshold to consider medication; the rest of the clinical assessment still matters |
| 27 to below 30 | A weight-related health condition is generally also needed for this pathway |
| Below 27 | Falls outside these usual adult weight-management thresholds; seek an appropriate health assessment rather than trying to reach a higher weight |

These thresholds are described by [NIDDK](https://www.niddk.nih.gov/health-information/weight-management/prescription-medications-treat-overweight-obesity). They are not a promise of approval, an insurance decision, or universal criteria for every GLP-1 indication.

## Calculate BMI without treating it as a diagnosis

BMI can be calculated as weight in kilograms divided by height in meters squared, or as weight in pounds × 703 divided by height in inches squared. Use an accurate height and current weight; avoid rounding either to move across a cutoff. The [CDC adult BMI calculator](https://www.cdc.gov/bmi/adult-calculator/) explains BMI as a screening measure that should be considered with other health information.

| Example measurements | Approximate BMI | Interpretation |
| --- | --- | --- |
| 5 ft 4 in, 180 lb | 30.9 | Meets the BMI threshold for consideration, not automatic approval |
| 5 ft 7 in, 180 lb | 28.2 | A weight-related condition and full assessment would also matter |
| 5 ft 10 in, 200 lb | 28.7 | A weight-related condition and full assessment would also matter |

You can use our [BMI calculator](/tools/bmi-calculator/) to prepare for a visit. BMI does not distinguish muscle from fat or describe your entire health history. When the result is close to a threshold, ask for accurate measurement and interpretation rather than deciding from one rounded value.

## Which health conditions are relevant?

Examples of weight-related conditions in the approved-product evidence include **high blood pressure, abnormal blood lipids and type 2 diabetes**. The clinician determines which indication and evidence apply to you. See the [Wegovy prescribing information, clinical studies](https://www.novo-pi.com/wegovy.pdf).

Bring diagnosed conditions, current treatment and available records. Symptoms such as snoring or a single elevated blood-pressure reading may warrant evaluation, but they are not interchangeable with a confirmed diagnosis. Prediabetes, PCOS, joint symptoms or other concerns should be discussed on their own merits, without assuming they automatically qualify you.

The [GLP-1 eligibility guide](/blog/glp1-eligibility-requirements/) covers the broader assessment. It is reasonable to ask what documentation is needed and why.

## BMI does not override safety

The formal contraindications and the other precautions in a medication label are different categories. Both matter, but labeling every concern a contraindication can mislead patients.

| Category for Wegovy | What to discuss |
| --- | --- |
| Formal contraindications | Personal or family history of medullary thyroid carcinoma, MEN 2, or known hypersensitivity to semaglutide or the product's ingredients |
| Warnings and precautions | Pancreatitis or gallbladder history, kidney risks related to dehydration, severe digestive symptoms, diabetes medicines and diabetic retinopathy |
| Severe gastroparesis | Wegovy is not recommended in people with severe gastroparesis; this is distinct from the formal contraindication list |
| Pregnancy planning | For weight reduction, the label directs discontinuation when pregnancy is recognized and at least two months before a planned pregnancy |
| Breastfeeding | Requires a product-specific discussion; instructions differ by formulation |

This is not a complete safety checklist. Tell the clinician about all prescription and nonprescription medicines, supplements, past reactions and planned procedures involving anesthesia or deep sedation. A history of pancreatitis needs assessment; it should not be mislabeled as a formal absolute contraindication. Likewise, this adult guide should not imply that every person under 18 is categorically excluded from all approved semaglutide treatment.

## If you are below the threshold

**Do not gain weight to qualify.** There is no benefit in waiting for your health to worsen.

Ask what is driving your concern now: a changing weight pattern, symptoms, a laboratory finding, trouble with eating, or a goal that could be addressed differently. A clinician can discuss nutrition, activity, sleep, other medicines and appropriate evaluation without promising weight loss or a prescription.

If you are already receiving treatment and your BMI has fallen, do not assume this starting threshold means you should stop. Continuing or changing care is a separate clinical decision that considers your history and current benefit.

## What to bring to an evaluation

- Current height and weight, and any recent changes you want to discuss.
- A complete medicine and supplement list.
- Known diagnoses and relevant records or laboratory results, if available.
- Prior weight-management approaches and what was difficult about them.
- Allergies and personal or family history relevant to the medication.
- Pregnancy plans, pregnancy or breastfeeding status.
- Questions about cost, follow-up and the exact prescribed product.

You can explain what you know and ask what else is needed. A responsible assessment may lead to more information being requested, a different plan, or no prescription. Completing an intake does not predetermine the outcome.

## Clinical suitability, coverage and product choice are separate

Insurance requirements may differ from clinical criteria. Ask about coverage directly and review [current pricing](/pricing/) rather than assuming a qualifying BMI settles cost.

A compounded preparation is not FDA-approved and does not acquire an approved indication simply because a patient meets a BMI threshold. Discuss why that exact preparation is being considered and what approved options are available. [FDA information on unapproved GLP-1 products](https://www.fda.gov/drugs/drug-alerts-and-statements/fdas-concerns-unapproved-glp-1-drugs-used-weight-loss).

Review [how REMEVi works](/how-it-works/), [semaglutide information](/semaglutide/) and the [medical team](/medical-team/) before deciding what to ask. The goal of the evaluation is an appropriate care plan, not an automatic approval from a number.

## Frequently asked questions

### What BMI is usually considered for semaglutide weight management?

Adult weight-management medication is generally considered at BMI 30 or higher, or 27 or higher with a weight-related condition. The clinician must also assess the exact product, indication, safety and your overall health.

### Does BMI 30 mean I will be approved?

No. BMI meets one screening threshold, but it does not settle whether a medication is safe or appropriate, whether more information is needed, or whether insurance will cover it.

### What conditions might matter at BMI 27 to below 30?

Examples include high blood pressure, abnormal blood lipids or type 2 diabetes. Their relevance and documentation need clinical review; a suspected diagnosis or a checklist answer does not guarantee eligibility.

### Should I gain weight to reach the cutoff?

No. Do not gain weight or change reported measurements to qualify. Ask for an accurate assessment and a plan appropriate for your health now.

### Do the same BMI rules apply to every semaglutide use?

No. Product indications differ, including uses unrelated to weight management. These adult weight-management thresholds also do not replace pediatric assessment or establish FDA approval of compounded preparations.
