Testosterone therapy is for appropriately diagnosed hypogonadism, not routine aging or fatigue alone. Diagnosis needs symptoms and consistently low morning testosterone; fertility plans and treatment risks must be reviewed.

## How the diagnosis is confirmed

Fatigue, reduced sexual desire and strength changes have several possible causes. The Endocrine Society guideline requires compatible symptoms and low levels confirmed with repeated morning measurements. The underlying cause, including possible testicular or pituitary disease, also needs evaluation. ## Discuss fertility before prescribing

External testosterone can suppress sperm production. The guideline recommends against starting it when near-term fertility is planned. Adding another drug should not be presented as a fertility guarantee; specialist assessment may be needed. ## Risks and follow-up

FDA requires warnings about increased blood pressure for testosterone products. Treatment also requires review of hematocrit, clinical response and prostate-related risks as appropriate. Monitoring reduces uncertainty but does not make treatment risk-free. ## What to expect from a consultation

Bring your medicines, previous results, symptoms and reproductive plans. Ask which specific improvement is expected, how it will be measured and when treatment will be reconsidered if it does not help. There is no guaranteed timeline for energy, strength or sexual function. Telehealth care must meet requirements in your location and may require testing or an in-person examination.
