Search "Trulicity vs Ozempic" and you will find a lot of pages that treat these two as near-identical products separated by a logo. They are not. They sit in the same drug class, they are both injected once a week, and they act on the same receptor, but at the molecular level they were built two completely different ways, and that engineering difference shows up in the trial data.

Here is the short version. Trulicity and Ozempic are both once-weekly GLP-1 injections FDA-approved for type 2 diabetes, but they are different molecules: Trulicity is dulaglutide, Ozempic is semaglutide. In the SUSTAIN 7 head-to-head trial, semaglutide lowered A1c and body weight more on average than dulaglutide. Neither is FDA-approved for weight loss. A clinician determines which fits your health picture. Individual results vary.

What follows is the science behind that summary, drawn from the current FDA prescribing information for both products and from the one large trial that put them head to head. Nothing here is medical advice.

## Two different molecules, one hormone pathway

Both medications imitate GLP-1, a hormone your gut releases after you eat. Native GLP-1 is a useful signal and a terrible drug: enzymes in your blood chew it up within a couple of minutes. Every GLP-1 medication is therefore an engineering answer to the same problem, which is how to keep a fragile gut hormone active in the body for a week instead of a few minutes. Dulaglutide and semaglutide answer it in opposite ways.

**Dulaglutide is a fusion protein.** According to the FDA label, the molecule is built from two identical, disulfide-linked chains, each one carrying a GLP-1 analog sequence covalently attached by a small peptide linker to the Fc portion of a modified human immunoglobulin G4 antibody. The GLP-1 portion is 90 percent homologous to native human GLP-1, with structural changes at the site where the enzyme DPP-4 would normally cut it. The whole assembly weighs roughly 63 kilodaltons. In other words, dulaglutide survives by being big. Bolting a GLP-1 analog onto an antibody backbone slows the body's ability to clear it, which produces an elimination half-life of about five days.

**Semaglutide is a modified peptide.** Its backbone is produced by yeast fermentation, and the label describes three deliberate changes: the lysine at position 26 is modified with a hydrophilic spacer and a C18 fatty di-acid, position 8 is altered to resist DPP-4, and position 34 carries a minor change so that only one fatty di-acid attaches. That fatty di-acid is the important one. It lets semaglutide bind reversibly to albumin, the most abundant protein in your blood, which shields it from filtration by the kidneys and from enzymatic breakdown. The molecule itself weighs 4,113.58 g/mol, roughly one fifteenth the mass of dulaglutide, and shares 94 percent sequence homology with human GLP-1. Its half-life is about one week.

Once either molecule reaches the GLP-1 receptor, the downstream biology looks similar. Insulin release becomes more responsive to glucose, glucagon output falls, and the stomach empties more slowly. Both labels note that the delay in gastric emptying is dose-dependent, largest after the first dose, and diminishes with subsequent doses. That last detail explains why the first few weeks on either medication tend to be the roughest. ## What the head-to-head trial actually measured

Most GLP-1 comparisons are indirect, stitched together from separate placebo trials. SUSTAIN 7 is the exception. Published in The Lancet Diabetes & Endocrinology in 2018, it was an open-label, parallel-group, phase 3b trial that randomized 1,201 adults with type 2 diabetes across 194 sites in 16 countries. Participants had an A1c between 7.0 and 10.5 percent while taking metformin, and they were assigned to one of four weekly injections for 40 weeks: semaglutide 0.5 mg, dulaglutide 0.75 mg, semaglutide 1.0 mg, or dulaglutide 1.5 mg. The primary endpoint was change in A1c; body weight was the confirmatory secondary endpoint.

On blood sugar, semaglutide came out ahead at both dose levels. A1c fell 1.5 percentage points on semaglutide 0.5 mg versus 1.1 on dulaglutide 0.75 mg, an estimated treatment difference of 0.40 points. At the higher pairing, A1c fell 1.8 points on semaglutide 1.0 mg versus 1.4 on dulaglutide 1.5 mg, a difference of 0.41 points. Both comparisons reached statistical significance for superiority.

On body weight, the gap was proportionally wider. Average weight fell 4.6 kg on semaglutide 0.5 mg against 2.3 kg on dulaglutide 0.75 mg, and 6.5 kg on semaglutide 1.0 mg against 3.0 kg on dulaglutide 1.5 mg. Individual results vary, and these are 40-week averages in a diabetes population, not a projection for any one person. It is also worth remembering that SUSTAIN 7 was designed to test blood sugar control, not to maximize weight change, and it used the diabetes dose range rather than the higher doses studied in obesity programs.

Tolerability was broadly similar. Adverse events were reported by 68 percent of the semaglutide 0.5 mg group, 62 percent of the dulaglutide 0.75 mg group, 69 percent of the semaglutide 1.0 mg group, and 74 percent of the dulaglutide 1.5 mg group. Gastrointestinal events were the most common in every arm. Of the 1,199 patients exposed to treatment, 87 percent completed treatment and 94 percent completed the trial.

One honest caveat: SUSTAIN 7 was open label, meaning participants and investigators knew which drug was being given, and the dulaglutide arms were capped at 1.5 mg because the 3 mg and 4.5 mg doses had not yet been approved when the trial ran. A trial run today would look different.

## Dosing, devices, and approved uses

The dose ladders are not parallel, which is why switching is never a milligram-for-milligram swap.

**Trulicity** starts at 0.75 mg once weekly, may increase to 1.5 mg for additional glycemic control, and can then rise in 1.5 mg increments after at least four weeks on the current dose, up to a maximum of 4.5 mg weekly. Each dose is a separate single-dose pen. It is FDA-approved as an adjunct to diet and exercise to improve glycemic control in adults and in pediatric patients 10 years and older with type 2 diabetes, and to reduce the risk of major adverse cardiovascular events in adults with type 2 diabetes who have established cardiovascular disease or multiple cardiovascular risk factors.

**Ozempic** starts at 0.25 mg once weekly for four weeks, a dose the label explicitly describes as an initiation dose that is not effective for glycemic control on its own. It then steps to 0.5 mg, and after at least four weeks at each level it may go to 1 mg and then to a maximum of 2 mg weekly. One multi-dose pen covers a dose range. It is FDA-approved to improve glycemic control in adults with type 2 diabetes, to reduce major adverse cardiovascular event risk in adults with type 2 diabetes and established cardiovascular disease, and to reduce the risk of sustained eGFR decline, end-stage kidney disease, and cardiovascular death in adults with type 2 diabetes and chronic kidney disease.

Note what is absent from both lists: weight loss. Neither product is FDA-approved for chronic weight management. Both carry a boxed warning about thyroid C-cell tumors observed in rodents, and both are contraindicated in people with a personal or family history of medullary thyroid carcinoma or with Multiple Endocrine Neoplasia syndrome type 2. Neither is for type 1 diabetes.

## Side effects, compared

Both labels report gastrointestinal effects as the dominant adverse reactions, and both report them as more common at higher doses.

In Trulicity's pooled placebo-controlled adult trials, nausea occurred in 12.4 percent at 0.75 mg and 21.1 percent at 1.5 mg, against 5.3 percent on placebo. Diarrhea ran 8.9 and 12.6 percent versus 6.7 percent, and vomiting 6.0 and 12.7 percent versus 2.3 percent. Gastrointestinal adverse reactions of any kind were reported by 32 percent at 0.75 mg and 41 percent at 1.5 mg, against 21 percent on placebo.

In Ozempic's placebo-controlled trials, nausea occurred in 15.8 percent at 0.5 mg and 20.3 percent at 1 mg, against 6.1 percent on placebo. Diarrhea ran 8.5 and 8.8 percent versus 1.9 percent, vomiting 5.0 and 9.2 percent versus 2.3 percent, and constipation 5.0 and 3.1 percent versus 1.5 percent.

Read those side by side and the pattern is the class, not the brand. Both slow gastric emptying, so both produce nausea, and both produce most of it during dose escalation. Persistent vomiting, severe abdominal pain, or symptoms that stop you from keeping fluids down are reasons to contact your provider rather than to push through, and both labels carry warnings about pancreatitis, acute kidney injury from volume depletion, and gallbladder disease. ## What they cost in 2026

List price and what anyone actually pays are two different numbers, and the gap has become enormous.

Eli Lilly's published pricing disclosure puts Trulicity's wholesale acquisition cost at $987.19 for a one-month supply of four pens, at every strength from 0.75 mg to 4.5 mg. Novo Nordisk lists Ozempic's wholesale acquisition cost at $1,027.51 per month across every dose and presentation. Those are the prices manufacturers charge wholesalers, before insurance, rebates, or discounts.

Cash-pay channels are far lower. Novo Nordisk's current self-pay price for Ozempic is $349 per month for the 0.25 mg, 0.5 mg, and 1 mg doses and $499 per month for 2 mg, with an introductory $199 per month for the first two fills of 0.25 mg or 0.5 mg for patients new to the program, offered through December 31, 2026. Lilly's self-pay price for Trulicity through its direct pharmacy channel starts at $389 per month at every dose. Both manufacturers also run commercial-insurance copay programs advertised at as little as $25 per fill, subject to eligibility rules that exclude government beneficiaries.

## How a clinician actually chooses

There is no universal winner here, and any article that names one is skipping the part that matters.

SUSTAIN 7 gives semaglutide the edge on average A1c and average weight reduction at the doses tested. That is real, and it is one input. It is not the only one. A prescriber is also weighing your A1c starting point, your kidney function, your cardiovascular history, whether you have had pancreatitis, what you tolerated on previous medications, whether a fixed single-dose pen or a dial-a-dose pen fits your life better, what your plan actually covers, and what happens when your coverage changes next January.
