Epitalon courses: what “intermittent courses” means
The policy’s wording, and the course lengths published studies actually used.
HHS’s 2026 policy lists Epitalon for injection “in intermittent courses” but does not define a course. Published human studies of synthetic Epitalon used single courses of 10 or 20 days; older Epithalamin studies repeated courses every six months. Epitalon is not FDA-approved, and REMEVi does not currently offer it.
Epitalon is not FDA-approved. Since HHS’s October 2026 interim policy, a state-licensed 503A pharmacy may compound it on a patient-specific prescription under listed conditions.
REMEVi does not currently offer Epitalon.
What the policy says about courses
HHS’s interim enforcement-discretion policy for 503A compounding (Federal Register, October 30, 2026) lists epitalon acetate for injection, in intermittent courses. Secondary reports note that the policy does not define how long a course is or how often it may repeat. That is left to the prescribing provider, within every other condition of the policy.

Published course lengths
| Study | Substance | Route | Course length | Repeated? |
|---|---|---|---|---|
| Korkushko et al., 2007 | Epitalon (synthetic) | Intramuscular injection | 10 days | Single course |
| Khavinson et al., 2002 (retinitis pigmentosa) | Epitalon (synthetic) | Beside the eye (not a policy route) | 10 days | Single course |
| Ivko et al., 2020/2021; Khavinson et al., 2021 | Epitalon (synthetic) | Under the tongue (not a policy route) | 20 days | Single course |
| Korkushko et al., 2006 | Epithalamin (pineal extract, not Epitalon) | Intramuscular injection | Five injections at 3-day intervals | Every 6 months for 3 years |
Online protocols often describe one or two courses a year. We could not trace a repeat schedule for synthetic Epitalon to a published human study; the repeated schedules in the literature come from Epithalamin.
How long should you take Epitalon?
No study has established a course length for Epitalon. Published human studies of the synthetic peptide used single courses of 10 days by injection or 20 days under the tongue, and HHS’s policy says simply “intermittent courses.” A licensed provider sets whether a course is appropriate and how long it runs; never self-dose.
How often can an Epitalon course be repeated?
There is no published human evidence on repeating courses of synthetic Epitalon. The six-monthly repeat schedules often quoted online come from older Russian studies of Epithalamin, a pineal extract that is a different substance. Long-term effects of repeated courses are unknown, which is a question to raise with a licensed provider.
