Which weight-loss pills are FDA-approved?

The most useful starting point is the exact medicine and formulation. A familiar ingredient name does not tell you whether a product is approved for weight management. This guide focuses on adult oral options; pediatric use and other indications need their own assessment.

These are long-term weight-management options used alongside nutrition and physical activity. The table is a starting comparison, not a complete safety checklist. Phentermine alone is different from Qsymia. Its approval is for short-term use, described in its label as a few weeks. Do not assume a combination medicine’s long-term indication applies to a single ingredient. Adipex-P prescribing information

Wegovy and Foundayo: two oral GLP-1 options

FDA approved Foundayo on April 1, 2026, for weight reduction and long-term maintenance in adults with obesity, or overweight with a weight-related condition. Describing Wegovy as the only approved oral GLP-1 option for obesity is now out of date. FDA approval announcement

The daily routine can help frame a discussion. Wegovy tablets are taken in the morning on an empty stomach with up to four ounces of water, followed by at least 30 minutes before food, drinks or other oral medicines. The same number of milligrams does not mean an equivalent dose across different products or routes. Do not convert oral and injectable doses yourself; switching requires the prescriber’s product-specific instructions. Wegovy label, section 2.5

What do the weight-loss studies actually tell us?

OASIS 4 randomized 307 adults with obesity, or overweight with a related condition, without diabetes. With lifestyle support, oral semaglutide 25 mg produced an estimated average weight change of −13.6% at 64 weeks, versus −2.2% with placebo, in the published treatment-policy analysis. These are group averages; participants had differing responses and some stopped treatment. OASIS 4 primary publication

Keep the analysis attached to the number: the current Wegovy label’s Study 7 table reports a −2.4% placebo change with its specified analysis, while the publication above reports −2.2%. Do not combine figures from different analyses. OASIS 1 studied 50 mg oral semaglutide, a different dose from the approved 25 mg Wegovy tablet maintenance dose. Foundayo’s label describes a separate 72-week trial in adults without diabetes: the highest approved dose group averaged −11.1%, versus −2.1% with placebo, in the intent-to-treat analysis. This is not a head-to-head comparison with Wegovy. Different populations, protocols and handling of missing data limit comparisons across trials. Foundayo label, Table 6

Safety questions that can change the choice

Your clinician needs a complete medicine list, including supplements, plus your diagnoses and pregnancy plans. Risks differ by product:

• Qsymia: pregnancy is a contraindication because of fetal harm. Ask about pregnancy testing, contraception, eye symptoms, mood changes and how to stop safely; abrupt withdrawal of higher doses can cause seizures. Qsymia label
• Contrave: chronic opioid use, uncontrolled high blood pressure and seizure disorders are among its contraindications. It also carries a boxed warning about suicidal thoughts and behaviors associated with antidepressants. Contrave label

• Oral GLP-1 products: both labels include thyroid C-cell tumor warnings and contraindications for personal or family medullary thyroid carcinoma or MEN 2. Gastrointestinal symptoms, dehydration and other risks need review. Wegovy label · Foundayo label

Get urgent medical help for severe persistent abdominal pain or inability to keep fluids down. Trouble breathing or swelling of the face or throat requires emergency care. Ask your prescriber which symptoms should mean stopping your specific medicine and calling immediately. The product labels above explain their individual warning signs.

Approved, compounded and supplement are different categories

FDA approval applies to a specific finished product. A compounded oral preparation is not Wegovy, Ozempic or Rybelsus, and is not an FDA-approved generic. FDA does not review compounded medicines for safety, effectiveness and quality before marketing. They should only be used when an approved medicine cannot meet the patient’s medical needs. Dietary supplements are another category. They do not undergo FDA premarket approval for safety and effectiveness. A “natural” label does not make a supplement an approved obesity medicine. FDA: dietary supplements

Bring these questions to your clinician

1. Which exact product and approved indication are we discussing?
2. What in my history or medicine list makes it suitable—or unsuitable?
3. How will I take it on a normal workday, and what should I do after a missed dose?
4. How will we assess nutrition, tolerability and response, and when would we change the plan?
5. What is the complete cost over the same treatment period, including visits and follow-up?
6. If a compounded medicine is proposed, what medical need cannot be met by an approved option?
