What to Expect During Your First Month on Semaglutide

An open journal and coffee for tracking the first month on semaglutide

The first month of weekly semaglutide is usually a gradual adjustment period at a starting dose. Appetite, digestive symptoms and weight changes vary, so early pounds lost are not a reliable pass-or-fail test.

Before the first dose

Confirm the exact medicine and instructions, record your starting weight and relevant health measures, and agree on a follow-up plan. Your clinician should review nutrition, other medicines and symptoms that need urgent attention.

The first weeks: assess tolerability

The starting dose allows gradual introduction. You may notice appetite changes, digestive symptoms or little change at first. There is no validated day-by-day or month-by-month weight-loss forecast for you. Do not increase the dose on your own to meet a scale target.

Over several months: look at the trend

Your care team can review weight, nutrition, activity, strength, side effects and relevant health measures. A few unchanged weigh-ins do not establish treatment failure. If progress stalls, check the full plan and tolerability before deciding whether to change treatment.

What the trials actually measured

STEP 1 studied semaglutide 2.4 mg with lifestyle intervention in adults without diabetes for 68 weeks; average loss was 14.9%, compared with 2.4% with placebo. SURMOUNT-1 studied tirzepatide for 72 weeks; the 15 mg group averaged 20.9%, compared with 3.1% with placebo. These are different trials and cannot supply a personal weekly target or establish compounded-product results.

Nutrition, movement and follow-up

A smaller appetite still needs adequate nutrition. Discuss protein, fiber, hydration and activity appropriate for your health and ability. Seek help for persistent vomiting, dehydration or severe abdominal pain. Track how you feel and function as well as your weight.

Plan for maintenance

Weight management may require continued treatment. Discuss long-term affordability, monitoring and what to do if you need to stop; tapering is not a proven guarantee against regain.
