In SURMOUNT-1 (NEJM, 2022), tirzepatide 15 mg weekly produced a mean body-weight reduction of −20.9% at 72 weeks, on the treatment-regimen estimand.

In STEP 1 (NEJM, 2021), semaglutide 2.4 mg weekly produced a mean body-weight change of −14.9% at 68 weeks, against −2.4% on placebo.

How is retatrutide different from tirzepatide?

In two ways that matter and a third that matters more. First, the receptors: tirzepatide acts on two (GIP and GLP-1) and retatrutide on three (GIP, GLP-1 and glucagon). Second, the stage: tirzepatide finished its phase 3 programme years ago and has FDA-approved brand products; retatrutide is still in phase 3.

And the third: tirzepatide can be prescribed today. Retatrutide cannot. A licensed provider can evaluate you and prescribe compounded tirzepatide; nobody in the United States can prescribe you retatrutide outside a clinical trial, and federal law does not allow a pharmacy to compound it.

Is retatrutide more effective than tirzepatide?

The published trials cannot answer that. TRIUMPH-1 and SURMOUNT-1 are separate studies with different populations, designs, durations and eras, and neither one included the other drug as a comparator. Setting their percentages side by side shows what each trial reported; it does not show what would happen if the two were run against each other.

The first trial designed to compare them directly is TRIUMPH-5, registered with an obesity population and tirzepatide as the comparator. As of September 2026 it has not reported results, and we will not publish an answer before one exists.

Retatrutide vs Ozempic: what’s the difference?

That question compares a molecule with a brand, so the names are worth sorting first. Semaglutide is the medication in Ozempic® and Wegovy®; tirzepatide is the medication in Mounjaro® and Zepbound®. Retatrutide has no brand because it has no approval: there is no branded retatrutide product in any country.

So the practical difference is the same one this whole page keeps arriving at: one of these molecules can be prescribed in the United States today and the other cannot.

The comparisons, answered.

  1. Nobody honest can say yet. Retatrutide is not approved, no application for approval has been submitted, and there is no product anyone can prescribe. What replaces what will depend on regulatory and clinical decisions that have not happened.

  2. There is nothing to switch to today: retatrutide is not available by prescription in the United States. If it is ever approved and available through our partner pharmacies, we will say so here. And either way, which medication is appropriate for you is decided by the licensed provider who reviews your history, not by a web page.

  3. Two of the three. Our licensed providers prescribe compounded semaglutide and compounded tirzepatide, prepared by state-licensed US 503A pharmacies under an individual prescription. We do not offer retatrutide.
