Missed a GLP-1 Dose? Check the Exact Product First

A missed dose is easier to handle when you start with the actual medication label. “GLP-1” names a class of medicines, not one catch-up schedule. Even products containing semaglutide can have different instructions.

The guidance below summarizes current U.S. prescribing information for the named branded products. It cannot establish instructions for a compounded medicine or decide how you should restart after a longer break. If you are unsure whether you already took a dose, ask your pharmacist or prescriber before taking another.

Write down four details first

Have the following information ready:

1. The full product name and whether it is a tablet or injection.
2. Your prescribed dose and the date and time of the last dose actually taken.
3. The date and time of the missed scheduled dose.
4. The date and time of your next scheduled dose.

Also note whether you skipped treatment because you were ill or having side effects. Missing a dose because a calendar reminder failed and stopping because of repeated vomiting are different situations to explain to the care team.

Wegovy injection: look ahead to the next dose

For one missed injection, the Wegovy label uses the time until the next scheduled dose. If that dose is more than two days away, the label says to administer the missed injection as soon as possible. If it is less than two days away, skip the missed injection and resume on the usual day. Ask your pharmacist if you are exactly at the 48-hour boundary or unsure of the timing. For two or more consecutive missed injections, the current label directs restarting dose escalation at a lower dose to reduce gastrointestinal reactions. Contact your prescriber to set that restart plan; do not choose the lower dose yourself. Older instructions you find online may differ from the current label.

Wegovy tablets: skip the missed tablet

Ozempic injection: count from the missed dose

For Ozempic injection, the label says to take a missed dose as soon as possible within five days after the missed dose. If more than five days have passed, skip it and take the next dose on the regularly scheduled day. Ozempic injection prescribing information, section 2.1

Notice the different reference point: this instruction counts time since the missed dose. Do not substitute it for the wording on your Wegovy prescription merely because both contain semaglutide.

Mounjaro and Zepbound: a four-day window

Both tirzepatide brands instruct patients to take a missed dose as soon as possible within four days, or 96 hours, after the missed dose. After more than four days, skip it and resume on the regular day. Their labels require at least three days, or 72 hours, between doses when changing the weekly administration day. These instructions address a missed dose. They are not a personalized restart plan after several missed weeks. Let the prescriber review a longer interruption before you resume.

Compounded medication needs its own instructions

For compounded semaglutide or tirzepatide, contact the prescriber or dispensing pharmacy with your exact label and dosing history. Do not assume a brand’s missed-dose window applies to your formulation. If you have a new vial, a different concentration, or instructions expressed in unfamiliar units, resolve that question before measuring medicine. A missed dose does not justify guessing a conversion or adding an extra amount.
