How much does Zepbound cost without insurance?

As of October 8, 2026, Zepbound without insurance costs $299 a month for the 2.5 mg starting dose and $399 for 5 mg through LillyDirect, Lilly’s self-pay pharmacy. The 7.5–15 mg doses cost $449 a month if you refill within 45 days, or $499–$699 if you don’t. The list price is $499–$1,086.37 per fill.

How much is Zepbound without insurance?

Quick answer: $299–$699 a month on LillyDirect, depending on the dose, or $449 for 7.5–15 mg if you refill on time (as of October 8, 2026).

Lilly sells Zepbound directly to people paying cash through LillyDirect, its online self-pay pharmacy. The price depends on the dose, not on whether you choose the single-dose vial or the KwikPen: both cost the same per month. Lilly defines a month as 28 days — four single-dose vials or one KwikPen — and says taxes and fees may apply. Source: LillyDirect Zepbound terms and conditions, checked October 8, 2026.

Most people start at 2.5 mg, which Lilly describes as a starting dose, not a maintenance dose. Your prescriber decides whether and when the dose goes up, and the price follows the dose — so budget for the dose you are likely to stay on, not for the first box.

What is Zepbound’s 45-day refill rule?

Quick answer: On 7.5–15 mg you pay $449 only if each refill is bought within 45 days of the last delivery; miss it and the price returns to $499 or $699.

Lilly calls it the Zepbound Self Pay Journey Program. Since February 23, 2026, the $449 price is applied automatically the first time you buy a 7.5, 10, 12.5 or 15 mg vial or KwikPen. To keep it, each refill purchase has to be completed within 45 days of the date the previous one was delivered or picked up.

If you pause, travel or run late and the gap passes 45 days, the next fill is charged at the regular price: $499 for 7.5 mg and $699 for 10, 12.5 or 15 mg. The rule does not change the 2.5 mg and 5 mg prices.

A practical habit: order the refill well before day 45. Shipping and prescription renewals take time, and the clock counts from the day the previous box arrived, not from the day you started it.

Is the Zepbound vial or the KwikPen cheaper?

Quick answer: Neither, on LillyDirect: the vial and the KwikPen cost the same per 28-day supply at every dose. Vials need syringes and needles, bought separately.

The single-dose vial is sold only through LillyDirect and only to self-pay buyers. Each month is four vials, and you draw each weekly dose with a syringe and needle you buy separately. The KwikPen holds the month in one pen and needs pen needles. Full detail is in Zepbound vials on LillyDirect: the self-pay price.

The single-dose autoinjector pen — the presentation insurance plans usually cover — is a third option. Its savings cards work differently, as the next sections explain.

Where else can I buy Zepbound without insurance?

Quick answer: Costco members pay $499.22–$699.21 per KwikPen; GoodRx shows coupons from $299; TrumpRx.gov points to LillyDirect’s prices. Checked October 8, 2026.

Costco’s public price search lists only the Zepbound KwikPen — no vial — at $499.22 for 2.5 to 7.5 mg and $699.21 for 10 to 15 mg at warehouse pickup, slightly more for home delivery. Costco does not say whether Lilly’s self-pay savings card can be used at its pharmacy, so ask before you fill.

GoodRx shows Zepbound “as low as” $299 for a 2.5 mg KwikPen with its coupon, next to an average retail price of $600.65. GoodRx did not show which pharmacy or location that price applies to, so check it with your own ZIP code.

TrumpRx.gov, the federal price site, lists the Zepbound vial at the LillyDirect prices (from $299) next to an “original price” of $1,087, and sends you to LillyDirect to buy.

Does the Zepbound savings card work without insurance?

Quick answer: Only one does. The single-dose pen cards need commercial insurance; the KwikPen self-pay card works without it, at $299–$449 a month.

Lilly’s terms describe three different cards. Which one applies depends on your insurance, not on your dose:

  • Commercial plan that covers Zepbound: pay as little as $25 for a 1-, 2- or 3-month fill of the single-dose pen. Savings are capped at $100 per 1-month fill and $1,300 a year.
  • Commercial plan that does not cover Zepbound: pay as low as $499 for a 28-day fill of up to four single-dose pens.
  • KwikPen self-pay card: $299 for 2.5 mg up to $449 for 7.5–15 mg (with the 45-day rule), up to 11 fills a year.

All three cards expire December 31, 2026, and none can be used by people with Medicare, Medicaid or other government coverage. More in Mounjaro, Zepbound and Wegovy savings cards explained.

Can I get Zepbound for less with Medicare?

Quick answer: Possibly. CMS’s Medicare GLP-1 Bridge sets a $50 copay for eligible Part D members through December 31, 2027, for certain GLP-1 drugs its page doesn’t list.

The Medicare GLP-1 Bridge is a short-term CMS demonstration running from July 1, 2026, to December 31, 2027. It sits outside the normal Part D benefit: the Part D deductible does not apply, and the copay does not count toward your yearly out-of-pocket total. CMS’s page does not name the drugs, so ask your Part D plan whether Zepbound is included and whether you qualify. Source: CMS: Medicare GLP-1 Bridge.

What should I check before paying cash for Zepbound?

Quick answer: Your likely maintenance dose, the device, the 45-day rule, taxes and fees, and your insurance — coverage with a savings card can cost far less than cash.

Five questions settle most of the cost:

  1. Which dose will I likely stay on? The 2.5 mg price is a starting price only.
  2. Vial or KwikPen? Same price on LillyDirect; vials add syringes and needles.
  3. Can I refill within 45 days every time? If not, plan on the regular price.
  4. Does my insurance cover it? Ask before paying cash; a covered fill with a card can be far less.
  5. Who handles side effects and dose changes? The price of the drug is not the whole cost of care.

Is there a compounded option?

Quick answer: Yes. REMEVi offers compounded tirzepatide, prescribed by licensed providers: From $225 per 4 weeks on the 24-week plan · or $269 every 4 weeks, cancel any time.

A compounded medication is prepared by a state-licensed pharmacy for one patient’s prescription. Compounded medication is not FDA-approved and is not a generic of Zepbound. The FDA does not review compounded drugs for safety, effectiveness or quality before they are sold. Brand names appear on this page only to orient you; REMEVi does not sell brand-name drugs.

At REMEVi, a licensed provider reviews your health questionnaire and decides whether a prescription is appropriate. Compounded tirzepatide: From $225 per 4 weeks on the 24-week plan · or $269 every 4 weeks, cancel any time. Each plan has one price at every dose and every renewal of that plan, with no membership fee, and you get a full refund if you are not prescribed.
